There are strict time limits to making a claim to a tribunal, and you will need to submit claim for ET1, no later than 3 months of the date of the incident.  Form ET1 is available from the Employment Tribunal Services website.

The form comes with information you will need to complete it, and details of which tribunal office to send your claim to.
